1. What are the primary medical specialties in which you were originally trained (e.g., pediatrics, family medicine)? Board Certified in Pediatrics

2. Approximately how many individuals with autism have you treated, and what is the age range? 250 - 300; ages 2 - 21

3. Do you usually have a waiting list? Yes, but usually email appointments can be accomodated within 1 - 2 weeks.

4. On the average, how long do you spend face-to-face with a new patient for an initial consultation? 1 - 1.5 hours; only one hour will be charged.

5. After the first consultation, what is the average length of an appointment? 1/2 - 1 hour; only half hour will be charged.

6. Are your patients advised to use special diets such as GF/CF, SCD, low-oxalate, etc.? Patients are encouraged to try GFCF first or given the option of having labs dictate which diet to implement.

7. Can you collaborate with other specialists we will be dealing with (e.g., gastroenterologists)? Yes, on an on-going basis.

8. Are you willing to collaborate on treatment and testing with a child’s pediatrician if he/she is receptive? Yes, I am happy to collaborate on the therpeutic process.

9. Do you or anyone in your office speak languages other than English? Chinese and Spanish

10. Are you willing to offer phone consultations for distant patients? Yes

11. Do you provide any form of after-hours support? Yes, direct cell phone connection with Dr. Lin after-hours.

12. Do you sell proprietary nutritional supplements or have a sales agreement with supplement suppliers? Do you sell supplements at list price or less? No, patients are provided different supplement recommendations to find the most suitable for their children.

13. Do you take insurance? If not, do you provide organized, coded insurance forms? Superbills coded with appropriate medical diagnosis will be provided to ensure maximum insurance reimbursement.

14. Do you bill for laboratory tests done by commercial laboratories? How do you break down the fees? Most labs are billable through insurance. Labs are selected according to cost-effectiveness (most information for least cost). Parents will work with selected labs for payment or insurance billing.

15. Is there a reason you might not bill for an office visit? Pro Bono or free consultations will be provided for TACA qualified or Generation Rescue qualified families with real financial difficulties.
