1. What are the primary medical specialties in which you were originally trained (e.g., pediatrics, family medicine)? Board Certified Family Practice. I am in a practice with 3 other FP doctors who do not do Defeat Autism Now! Treatments, but are very supportive.

2. Approximately how many individuals with autism have you treated, and what is the age range? Nearly 400; one year to 20's.

3. Do you usually have a waiting list? Yes

4. On the average, how long do you spend face-to-face with a new patient for an initial consultation? I ask parents only to come to an evening group meeting where I spend an hour talking about the biomed approach, so I know that they have a certain fund of knowledge and are willing to make diet changes, and understand what to expect then 1 hr for Q&A

5. After the first consultation, what is the average length of an appointment? Appointments in the office are 45 minutes long after that.

6. Are your patients advised to use special diets such as GF/CF, SCD, low-oxalate, etc.? Always. Start with GFCF, then most move to SCD. If they have an oxalate problem, then go low oxalate. All are to be preservative, dye, sugar freee and organic and grassfed/free range meat when possible.

7. Can you collaborate with other specialists we will be dealing with (e.g., gastroenterologists)? I send to neurology if I suspect seizures and to GI if I am not getting a handle on the GI issues. I use local doctors, but have turned to Dr. Krigsman in a few cases.

8. Are you willing to collaborate on treatment and testing with a child’s pediatrician if he/she is receptive? I encourage patients to stay with their primary doctor, if they are able to and have a doc they like. If not, I offer them to see one of my partners as their PCP.

9. Do you or anyone in your office speak languages other than English? No.

10. Are you willing to offer phone consultations for distant patients? No.

11. Do you provide any form of after-hours support? One doctor is on call for questions whenever the office is closed. That would be me, one of my partners or a 5th doctor that works with us. If I am not on call, they can usually reach me if needed.

12. Do you sell proprietary nutritional supplements or have a sales agreement with supplement suppliers? Do you sell supplements at list price or less? Yes, I sell supplements at list price. We use Kirkman, Thorne, and Nordic Naturals, mainly.

13. Do you take insurance? If not, do you provide organized, coded insurance forms? Yes, all but Medicaid

14. Do you bill for laboratory tests done by commercial laboratories? How do you break down the fees? No, but we do lab draws here.

15. Is there a reason you might not bill for an office visit? Generally no.
