1. What are the primary medical specialties in which you were originally trained (e.g., pediatrics, family medicine)? Family Practice

2. Approximately how many individuals with autism have you treated, and what is the age range? 40 +, Age range 2.5 - 23 years

3. Do you usually have a waiting list? No

4. On the average, how long do you spend face-to-face with a new patient for an initial consultation? At least one hour. An extensive history horm and resource list is mailed to the family prior to the first appointment

5. After the first consultation, what is the average length of an appointment? 30 minutes to 1 hour

6. Are your patients advised to use special diets such as GF/CF, SCD, low-oxalate, etc.? Yes

7. Can you collaborate with other specialists we will be dealing with (e.g., gastroenterologists)? I am in a rual area, so I can collaborate, but it is a long drive to the closest Peds GI.

8. Are you willing to collaborate on treatment and testing with a child’s pediatrician if he/she is receptive? Yes

9. Do you or anyone in your office speak languages other than English? No

10. Are you willing to offer phone consultations for distant patients? No, I have an email system set-up for established long distance patients. I am not set-up to bill for any contact other than in person for now.

11. Do you provide any form of after-hours support? Yes, Pager, email.

12. Do you sell proprietary nutritional supplements or have a sales agreement with supplement suppliers? Do you sell supplements at list price or less? I usually have families buy directly from Kirkman or others. I sell a very small amount of product in my office; mostly probiotics at list price or less.

13. Do you take insurance? If not, do you provide organized, coded insurance forms? Yes, with companies I am a participating provider. The coded insurance form is provided if I am non-participating. I cannot accept any form of Medicad at this point (Michigan)

14. Do you bill for laboratory tests done by commercial laboratories? How do you break down the fees? No. We distribute the kits and order forms at the office and the patient/family provides the insurance inforamtion or payment directly to the Lab with the completed test kit.

15. Is there a reason you might not bill for an office visit? I bill for my work and that of my staff. Insurance usually covers the visit we do so much communication with families over the phone and email taht is not charged/billed
