1. What are the primary medical specialties in which you were originally trained (e.g., pediatrics, family medicine)? Family Medicine

2. Approximately how many individuals with autism have you treated, and what is the age range? 250

3. Do you usually have a waiting list? Yes, approximately 4 to 6 weeks

4. On the average, how long do you spend face-to-face with a new patient for an initial consultation? 1.5

5. After the first consultation, what is the average length of an appointment? 45 minutes (range 30 to 60 minutes)

6. Are your patients advised to use special diets such as GF/CF, SCD, low-oxalate, etc.? Yes, all diet types depending on where the patient is at in terms of their efforts.

7. Can you collaborate with other specialists we will be dealing with (e.g., gastroenterologists)? As an ND in Ontario I can oly send the patient/family back to their GP/Pediatrician with direction to advocate.

8. Are you willing to collaborate on treatment and testing with a child’s pediatrician if he/she is receptive? Yes, absolutely.

9. Do you or anyone in your office speak languages other than English? No

10. Are you willing to offer phone consultations for distant patients? Yes

11. Do you provide any form of after-hours support? Yes, always, as needed for patients if the need is anticipated.

12. Do you sell proprietary nutritional supplements or have a sales agreement with supplement suppliers? Do you sell supplements at list price or less? For supplements developed for ASD parents are advised to order direct or from a Canadian distributor (if available). Our office does sell certain supplements, at retail prices or lower (depending on the supplement).

13. Do you take insurance? If not, do you provide organized, coded insurance forms? We don`t normally deal with insurance but have in the past, especially if the insurer accepts long term treatment plans. Insurance coding can be provided.

14. Do you bill for laboratory tests done by commercial laboratories? How do you break down the fees? Depends on the lab - some labs will only accept payment from our office rather than patients. The patient is charged the physician`s rate plus an administration fee. Other costs like shipping are broken out separately at cost.

15. Is there a reason you might not bill for an office visit? Visits not completed/not responding to treatments etc...
